Why is my form disabled even though I have enabled it from going to the Preferences?

    I have created a form for my website. I have been into the preferences and checked that the form is enabled. Then I embedded the code of the form onto my website via Microsoft Frontpage 2003. Then when I test that the form works by clicking on the submit button a message keeps popping up saying that the form is disabled.

    Can someone please help me?

    Your form was automatically suspended by our system because our anti-phishing scanner detected some malicious fields on it. I can see that you were asking username and password in your form, this type of fields are not allowed as this are obviously used in phishing. Also, Forms created with JotForm does not have the feature to interact with databases, there are other applications suited for that kind of job.
