    I'm sorry for the trouble caused to you.

    I did check this form and did not find any issue with it. It seems to be a simple form which collects name, address, email, phone and age. It is not asking for any sensitive or confidential information from users.

    Can you please let us know where this form is embedded? Please share the web page URL and we will take a look.

    Here is the email that it was embedded in, I have sent it as a photo
    and the link names:

    If I understood you correctly someone has sent you email with the link of the mentioned form and you think that it is the spam. Correct me if I'm wrong?

    JotForm in online form builder where every user can build a form. And form that was sent to you, was build by one of our users, but we can not influence on what our users will sent via their email or where they will share their forms. 

    We can take actions only if the form is build that against out Terms of use. As mentioned by my colleague it is a simple form and there are not fields that are asking for sensitive or confidential information from users.

    OK, thanks for the explanation.  I believe they are the first step
    in the scam, as there is a ton of this "secret shopper" spam going
    around right now.  I will bet you that this is one of the following,
    but I can't prove it without getting stung.  Thanks anyway for your
    attention to the issue.  The following is the modus operandi:

    You get an email offering “secret shopper” jobs with retailers
    like Wal-Mart, Kmart, Best Buy or Home Depot. If you click
    through to the website, it looks like you’re on a retailer’s
    site – but you’re not. You’re asked to provide some personal
    information to get started, and told you’ll soon get a cashier’s
    check for around $1,500. You’re instructed to deposit the check
    into your account to “activate” your employment, keep $300 of
    that money as “advance payment” to cover initial expenses, and
    wire back the rest.
    Problem is, the “job” isn’t real and it’s not associated with
    any actual stores. You’re dealing with a scammer. That check is
    a fake. If you deposit it, you’re on the hook to pay the bank
    back.
