Conditional logic - hide/show fields and send email to specific email address

    hi - I need to create an online form that I can display within an mobile application. The form will need to have a list of property reference numbers to choose from - and when a specific ref number is chosen it loads an email specific to that client for the form to be forwarded to on submission.

    The form would need to choose from "occupied" & "unoccupied" then open different form fields upon answer.

    It also needs conditional statement's - so for example if you answer "yes" to a question a comment box & submit image box appear.

    Also "add bedroom" would add a number of sub fields fo each bedroom.


    is this all possible before I crack on with trying to create one - thanks - paul

    Yes it is all possible to achieve that with JotForm. 

    If you want that your form is display properly on mobile devices you would need to make your form to be responsive. You can make it responsive in by adding Mobile Responsive Widget to your form.

    To send Email Notification to specific email address based on some selection inside of your form, you would need to set conditional logic to Send an e-mail after submission.
