Please be noted that if you want to open your form in Lightbox, you will need to add your form's lightbox embed code which is actually javascript code. A lightbox cannot be loaded just from a hyperlink.

If you just want to add a link to your form, you need to grab your form's direct URL and add it in the field "URL do website". Please check the screenshot below on how to grab your form's direct URL:
