    We are trying to find a platform that will allow generation of a continuing education certificate. For example, if a participant filled out a survey stating he/she had attended six hours of continuing education, upon completion of the survey, the platform would automatically generate a certificate with the name and 6 hours continuing education on the certificate. We have contacted a few companies but would like to have several to compare.

    Would your company be able to provide such a service?

    hi Diane

    my name is Randy & i'm a JotForm user. 

    from what i gathered, you're looking for a way to automatically generate certificates with all the necessary data (name, date, course, duration) from the participants. 

    fig. 1 shows the flow of the automated certificate distro.

    i entered 4 dummy data in the JotForm form (fig. 2). you can also see that in JotForm, you can program the courses to automatically generate the duration (in hours) of each item, not needing the manual input of the participants. 

    *note: there are other methods available but the one i used here are all based on Free Web Apps / Services. if there are users who have an alternative way of achieving the end result, please help out Diane. 

    i'd like to let you know that i was able to achieve what you want by doing an experiment using: 

     JotForm (for individual registration by participants or bulk registration by an admin)

     Google Sheets (Integration from JotForm) & Autocrat (mail merge add-on)
    [Free Account]

     Google Docs (to create certificate templates)

    [an automated thank you email with the attached certificates of each attendees can be saved & delivered to their email address as PDF or Google Docs format at a preferred time].
