Can I create a form, and if the respondent answers a given question a certain way, another field is displayed immediately below that question? In other words, one field asks if your spouse is traveling with you; if you say yes, another field appears that says: Name of Spouse:______________.

YES! and I have seen a bunch of online forms services and JotForm provides one of the better interfaces. Easy and intuitive! So you are in luck! You can dynamically show and hide a field on a form page and also lead the user to a different page based on the value of a field. Look for the Setup & Share tab when designing your form. Then look for the Conditions icon in the resulting toolbar.
