My form is working and getting submission- however - I do not get notified or the application forwarded to me?
How do I ensure that I get the form forwarded?

I took a look at your jotform and based on our logs it seems that the emails are being correctly forwarded to your account.
I did see however that you have used the Test Email button today, and since neither of the two emails in the email notification are the same as the email of your account, I believe that you were actually expecting the email notification to go to the emails specified.
