I have added a radio button field to a previously working form. After saving, when I load the form in my website the field renders but then disappears. There are no "hide field" conditions related to this field.
In addition to the URL, I am attaching a screenshot of the for as laid out in Jotform.

When I visited the page, I was able to view the form properly on IE, Firefox, Opera, and Chrome. Can you please share with us the browser and the version that you're using?
If you're having issues with your form showing up on your web page, you can try deleting your browser's cache and then reload the page again. Please let us know if you need further assistance on this matter.

Just to clarify: you ARE seeing the "Is This A..." radio button field? When I load the page, I see that field draw and then immediately disappear. The rest of the form renders just fine.
I am on Mac OS X 10.6.8, and confirm this behavior in Safari, Chrome and Firefox. I have emptied the cache in each and restarted all three browsers and the problem remains. Further, I checked IE and Firefox on a Windows 7 machine and got the same result- so please confirm that you are actually seeing the radio button field named "Is This a:" renedr and stay rendered.

I see now what you mean. No I'm not seeing your "Is this A:" field, but I also noticed that at the end of your conditions list, there is one that is set to show it only when the Upload file(s) field is filled. Deleting this condition will have your "Is this A:" field to appear.
Please let us know if this condition is now necessary as this is I believe a different bug that should be resolved but as far as I understand, is not relevant to this topic.

OK, I see that- should have check for it. Fixed, but problem is I never put that condition in there. Thanks for finding it!

It's good to know that your issue is now resolved. Need not to worry about it as well. We love helping everyone here in JotForm and we're more than happy to see people ask questions :)
As for how a condition was put there, I'm not sure how to explain that. But one thing I'm sure at is that I will have to report the bug that I noticed from your form earlier :P

Yeah, I'm not sure what conditional response one would want from Upload Files. Doesn't mean I wasn't screwing around with it, though. Anyway thanks for your help, this is a wonderful programming tool.
