why am getting "form disabled" message

    I checked your account and it seems that your account was automatically suspended by our Phishing Detector. Jotform has a tool called Phishing Detector which allows automatic suspension of forms created for phishing activities. I checked your forms and I see on one of your forms (http://www.jotform.us/form/51630470228146) have username and password field which is against our terms of use. And, also I noticed that you have added Credit Card Info field which is not allowed in JotForm.

    I have activated your account. I have also removed the fields that I mentioned to avoid your account to be suspended again. Please avoid having username and password field in your form and specially credit card info fields.
