I use Jot Form for employees time cards online as they are out in the field and they are to turn in their forms every day. There has always been a problem with using an I Phone 4 - the older version. Two of the employees use an I-phone 4 and the form either doesn't work or they are not receiving the email confirmation. I tested it on an I-Phone 4 and they are correct. I cannot submit form. I have added a widget and it works with tablets and androids no problem. I am stumped as to what to do.

I took a look at your website and I can see that you are embedding your form over js embed option. This code is great, but it is susceptible to conflicts and various other issues, amongst one of them being that it is not working great on mobile phones (some of them - depending on few different things).
Now what I would suggest is to try to open the form directly over the same iPhone 4 devices and see if that has the same issue: http://form.jotform.us/form/51604582546154
