I'm building a form for a friend and i wanted all the radio buttons in the entire form to be read only so the user can not tamper with the pre-checked option (is their a short script to do that because i have many radio butons). I just want user to fill in text box below and submit. Thus, i should get an e-mail reporting the pre-checked radio button fields and the users inputed data from the text boxes. You can set the radiobutton property to be read-only,
Regarding the notification, you can set a custom email in the email notification settings
