Can anyone please please help me figure out why custom lead fields on salesforce are not showing up as options to be mapped to jotform fields? Field types that are not mapping include email fields, radio button/checkbox fields, and picklist fields.
I really need help with this! In the guide to Mastering Salesforce Integration that was published recently, it says you should be able to integrate text box fields but they are not showing up! The Salesforce list does not include all of the custom lead fields I made.

I have tried to reproduce the issue with a test Salesforce Developer's account, but the field mapping seems to be fine on my side.
The next custom fields were added.
Then mapped via JotForm.
I have noticed two issues there, the Check Box field is supposed to have a 'True' value. And our multiple-selected values are forwarded as "red, green, blue" while the Salesforce format is "red;green;blue".
Can you please provide us with the screenshots of your custom Salesforce fields and selections on the Salesforce mapping step?

Thank you so much for responding! Here are the screenshots:
Please let me know if you need any further screenshots. I hope we can get this figured out! As you can see some of the custom fields are not showing up in the map form fields list.

Well, I also cannot replicate this and I'm not sure if the number of custom fields is causing it. You have a few.
Are you using your current Auth Key to some other apps? Because if not, try to reset it and use the new key in the integration. The last time I checked Salesforce, there was a setting to include the field in the group and I can't seem to find that.
The other fields are showing up based on your screenshot so maybe this could also be related to the field settings. In some of the field, there's a "General Options" section. Did you change some of them there?
Lastly, field type should match to each other so if you have a Textbox field in the form, you should also use Textbox field in the Salesforce custom field.
We can forward this thread to our developers, just in case.

Is the Auth Key the Security token or something else? I have tried resetting the security token already and it has not helped the situation. Many fields are still missing.
I have tried and made sure that the field types match and to look and see if there are any weird settings for the fields, but there are not.
Any word from the developers on this issue?

Yes, security token.
I have now forwarded this thread to our developers for further investigation. If there are updates, you should know it in this thread.

Has this issue been resolved yet? I'm facing a similar issue. I previously had custom fields mapped that now show up with blank labels on the Jotform mapping screen. However, the data still flows through from Jotform to Salesforce when I run a test. When I attempt to integrate a separate form, those fields with blank labels are not available on the pick list.

BostonSchoolsFund, I moved your reply as a new thread here: http://www.jotform.com/answers/637685. Will take a look at it after this thread.

This issue has not been resolved yet. Any updates from the developers?
- JotForm SupportdavidAnswered on August 13, 2015 at 01:32 PM
Unfortunately, there are no updates as of yet. As soon as they get a chance to look at this they will update you here.

Have there been any updates on this issue? The custom lead fields are still not showing up in the Salesforce integration. I have even doubled checked that the field types are consistent between Jotform fields and Salesforce fields but they still are not showing up. What else can I do?
- JotForm SupportdavidAnswered on November 19, 2015 at 01:20 PM
Though there are still no updates as of yet, you may want to consider using Zapier for the integration instead:
They have several Zaps already set up to create leads in Salesforce using your JotForm form.
Since I cannot say when they may be completely fixed, that may be your best bet.

Sorry for the late notice, but may we know if you're still having this problem? If so, would you mind integrating a JotForm to your Salesforce account and let us know which form it is?
This is so we can start working towards the resolution of your case.

We are still having this problem. I just reintegrated the EVYP High School Program Application to Salesforce. Also is there a way for the integration to save itself even if the Salesforce security token resets itself? The security token resets regularly and when you have many custom fields as we do, it is very annoying and tedious to redo the integration every time for each of our forms.
Please let me know if there is any progress on this issue. It has been unresolved for over a year and I would really like to be able to integrate our Jotforms to Salesforce automatically.

Thank you for the update. I will forward this to the developer assigned. Sometimes tickets go unnoticed when there is no action for a period of time. Our apologies.
As for the security token, if the token changes in Salesforce, unfortunately, it would need to be manually updated in JotForm.

Are the screenshots of your SalesForce custom fields in this thread still current? Is it mostly the checkbox-type custom fields that are missing?
