Hello,
I am writing in reference to our survey "Quarterly Duty Hour Log 04/01/15-06/30/15. There is a condition wizard for this survey and it doesn't seem to be working consistently. If you select "Yes" for the first question, it is suppose to open up more questions and so far, it's only worked for one of my programs. The other submissions who answered "Yes" for question 1 did not open up the other questions. It doesn't matter which browser they are in either. Please help ~ this is a very important survey.
Thank you,
Carol Lane

Hi Carol,
Can you please test my form first http://form.jotformpro.com/form/52106465101947?
Please tell us if it has the condition rules you wanted to happen or not..
You can also just clone my form so that you can review it on your Form Builder.

We will wait for your response.
Thanks.

WashUGMEReplied on July 31, 2015 at 8:24 AMGood Morning,
This worked for me but it’s not working for some folks in our institution. It’s really hit or miss. I asked them to use a different browser but that didn’t seem to help either. Any other ideas?
Thank you,

So what my colleague shared with you is the desired behavior? However, it worked for you but not for your colleagues in the institution?
I just tried it and works properly:
What browser version and operating system are they using? Maybe JavaScript is not enabled on their browsers: http://enable-javascript.com/
