Submission page says "404" error- how can I fix this?

Multiple people have reported that when filling out a jotform, they receive a "404 error code" when they push the submit button. Is there a way to fix this/why is this happening?

I did check your last edited form "SOM - Public Health Track Application" and did not find any issue with that. You have configured it to redirect users to custom thank you URL which seems to work fine and does not display 404 page error. The user is redirected to following URL:  http://pmch.utmb.edu/education/som-public-health-track 

Please let us know with which form you are having this issue with. Alternatively you can also let us know you web page URL where you have embedded your form and we will take a look.
