do you offer a service to create forms?

Would like to have my core forms designed for us.

We do not offer a paid service to create forms.  We do have an option to request a form to be created:

Click on "Create Form" from the "My Forms" section of your account, then click "Request".

There is no guarantee these will be made.  New developers create them when they have spare time in order to get used to the software and there are a lot of requests.  Many of them are created to spec though, so it never hurts to try.
