Incomplete values error: field is required

    Hello Jotforms Team,

    Suddenly my form at 

    http://form.jotform.us/form/51536523547154

    suddenly provides the following error when submitted.

     

    Incomplete Values
    There are incomplete fields in your submission: Contact First Name is required Contact Last Name is required Salutation is required Street Address 1 is required City (if international, list other principal subdivision, such as province, state or county): is required Postal code: is required Address Type is required Main/Preferred Phone is required Flat Fee Amount is required Please go back and fix the problem(s)
     
     
    This form has been working for weeks now with no issue and suddenly has stopped working.
    Can you please help?

    We are sorry for the inconvenience that you are experiencing. I have tried replicating the issue, and i was unable to replicate it on my end. I have tried submitting on this form:

    I have filled only the fields that were marked with the red asterisk as required.

    Submission was successful, and I haven't seen any errors.

    You can see this test submission on your submissions page. Can you please try testing that form again, to see if you are still able to replicate the problem. If it continues, please try clearing your browser cache, and then try submitting the form again.

    I have also cleared your form caches, in case it helps. Please let us know if you are still experiencing the issue.
