    Hi chulaong,

    To confirm, you would like to have your checkbox next to the label of your field? Unfortunately there is not an option to do that. However you can use the checkbox field and hide the label using CSS code.

    I created a sample form from a cloned version of your form and made the changes to get the desire effect. You can view the form here: http://form.jotformpro.com/form/52184291170956

    I am really sorry that our supporter has not described the solution but instead provided a clone. That's unacceptable. 

     

    Anyways, here is how you can clone the form to your account:

    Hi cubulaong,

    You can also inject the following code to your current form:

    #label_19, #label_17 {

    visibility : hidden;

    }

    I edited the option for the check box field and removed the text from the label. I also aligned the label to top.

    I'm in trouble. I cloned the form and did a ton of work on it. Now when I
    go to My Forms, the Clone flashes on the screen but doesn't let me click on
    it. Can you please see what's going on?
