Why is my one IMPORTANT form going into trash each day or so?

I have one form, a copy center request form, and every other day it goes to trash.  We have a paid for upgraded account.   Please assist.  I need this form to be available.  

** please note that I have just restored this page... again, and it is available for now.  On the page with the link provided below the it is the first item on the list... Copy Center Work Order Form new

The system is not supposed to move any forms to the Trash folder by itself. Can you please check your Account History page to see if there is any suspicious activity?
