Form Submission is not integrating with Constant Contact

Hello,

I set this form (http://www.jotform.me/form/52071864183456) up for automatic integration with Constant Contact.  But submissions made on this form are not transferring to Constant Contact.

This form is being used on the following website: http://www.sturmfinancial.com/p/order-form

Was the form embedded using its source code? 

Can you please try first re-embedding the form again to the website using instead the default embed method.

Make sure to clean/clear first any previous embedded form script on the webstie before you re-embed the form again.

After re-embedding the form, you can check if new incoming submissions are now sending the contacts to the Constant Contact integratin.
