    1. Should I set up a new url for this product when I purchase the license?

    2. Will this be the complete website? Can I use a sub domain? ex myforms.mywebsite.com

    3. I do not have my own server, I use a hosting company to host my websites... Will this be a problem

Thank you for your interest in our services.

    1. You can install it on existing domain.

    2. Yes, you can use subdomain.

    3. I see that your site is on Apache. Most probably JotForm Application will work with your hosting provider. However, we would like to recommend you to contact your hosting provider and check the JotForm Application Requirements with them:

    http://jotformdemo.interlogy.com/README.txt

    - Apache modules: rewrite and mime
    - Apache configuration for root directory should have "AllowOverride All"

    - PHP Configuration should enable short php open tags "short_open_tag = On"
    - Mysql 5 or higher
    - Zip and Gzip

Do you have a hosting company that you recommend that works well with this application?

For the main requirements, I believe Godaddy's shared hosting service has these features, or some that are turned off on the global level, but can be activated via .htaccess or php.ini.

Unfortunately, they don't have the V8 (*), wkhtmltopdf (*), and PHP APC.  These optional features should be installed on the root level of the server so you might need to purchase a dedicated server so that you can install these extra features.

I will also address your question to our developers if they have knowledge of web hosting services that allow all of these features without requiring you to acquire a dedicated server.

    Thank you for your patience.

(*) - Needs confirmation with the hosting service provider
