    Hi Rodolfo,

I think you are looking for a custom domain instead of URL. To accomplish this you would need to configure your DNS on your server. Please check our user manual.

    http://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform

You will also see a nice little video on how to accomplish this. If you like to modify the URL so it looks something like

    http://www.jotform.com/id/1234567890

You can check review this page.

Please let us know if this helps.

Thank you for the reply, I tried using the option to modify the URL from the survey tool, but I get this instead

    www.jotformpro.com/rodolfosiles/pm4dev

The system ads automatically my name in the form before the text I need to include in the URL

    What I need is this

    www.jotformpro.com/pm4dev

    Is this possible?

    Thank you for your help

The link http://www.jotform.com/id/1234567890 gets me to a 404 error page, But I found the video in this link:

The video instructions were quite good ( too bad the video doesn't have a slide to go back, you have to wait until it ends and see it all again.

I changed the DNS and now I can add the URL forms.pm4dev.com to all forms

    Thx!

Glad you were able to solve the issue. Regarding your question. If you customize the URL, this will  include the username.
