How do i put the form i created on mywebsite?

    Hi I searched for a contact number as i am not tech "savvy" and I need laymen's term tech support with using your product on my wix site.  Can someone contact me if possible at 1.855.488.1774 ext 9 time zone EST.

    I provided the link below as i was having a hard time embedding the code on my website.... so i chose the pDF way with the submit button but the button does nothing on my wix site... i need help please.
