New form added to Xara website, not showing up

We have created a new Feed form and added to our website built by Xara Web Designer Pro 11. Form widget works great in design mode, looks great in preview mode, but not showing up when loaded on web server. You can look at www.genquestdnalab.com/2  -  Contact page. You'll see a large white space in the lower right corner where the form should be. Suggestions on  how to fix this missing form?

I've found the form here: https://genquestdnalab.com/2/contact.htm. Your website runs with HTTPS and your form is not, that is the reason why the form is now showing up in the page. 

You have to re-embed your form and use our SSL method to fix that: https://www.jotform.com/help/63-How-can-I-receive-SSL-Submissions.
