    The submit button does not appear on my phone (landscape or portrait) - S4 with Chrome. Why

    I took a look at the page you have given the link for and I can see the same as you - part of the form missing when viewed over mobile devices.

    Now the reason why this happens seems to be because of the js embed method.

    What I would suggest is to actually use iframe embed code, which you can see how to get here: How to get the jotform iFrame Code

    Iframe embed is much better for mobile phones for multiple reasons and as such it will help you to resolve the issue.
