Where did the "text box" option go?

I need to be able to provide information at the top of my form and the "text box" option seems to have disappeared. Is there a new way to do this? 

Text and Text Box are both under Form Tools, whichever you want. 

I think you are referring to the Text tool, anyway they are both available.
