Unable to submit: Incomplete values

    I am having an issue with one of my forms...

     

    When trying to test my form, after I submit it, I get the below response.

    Incomplete Values
    There are incomplete fields in your submission: is required Please go back and fix the problem(s)

    I'm sorry but the form URL that you shared cannot be found. You probably deleted it.  

    Normally, the message Incomplete Values will appear if not all required fields are answered.
