Can I add a file to a form? I want to be able to add a PDF files as a reference for employees to use when filling out the form.

    Yes you can embed PDF file to your form to add a file URL.

    If you want to add a PDF URL, you can use Text tool. It's under form tools.

    However, if you want to embed the PDF on your form so your users can view it straight from your form, use this widget: http://widgets.jotform.com/widget/pdf_embedder
