I am trying to create a form with dependent drop downs....

My issue is this...

I am creating a form that needs to work like this:

Questions:

Year

Make

Model

When one is selected the the questions below need to match.

ie 

If the year is 2015 I need to see the makes associated with that year

then..

I pick the model

i now need to see all models associated with that make for that year

my issue is...

I want to export this using the integration plugin to google sheets

When this happens I end up seeing many headers that I dont need in the sheet populated by the form questions

But those extras questions need to be there for me to control the data.. help?
