    How can I add a checkbox that says "I agree to the terms" and then the submit button appears

    I do know and see that this has been discussed but the link that was up on the page is not doing it correctly. The URL that says go here for this example also does not work when I put it in the address bar. Please assist

     

    I would also like the terms to open in a new window if they click to review the terms before they click agree.
