Unable to submit form. Error message "502 Bad gateway"

I'm sorry for the trouble caused to you.

I did test your form and I am able to replicate the issue you are having. I clicked on the submit button but it just says "Please wait.." and then after a while it displayed "502 Bad gateway"

The issue seems to be caused because of the google spreadsheet integration and phone number field. Our back end team is already aware of this issue and they are looking into this. We will get back to you as soon as this issue is fixed.

integration is, I dont think I have one integrated to this form.
