I can't get my form link to work-

I am trying to add my form link to my group page on Facebook and it says form currently unavailable when you click it. I updated the old form to have the current info and even tgried clicking the old link on our page that I know worked because I recevied numerous submissions through it but even the old link won't work. Please help :)
