    Hi,

Can you try to re-embed your form using the iFrame embed method? To get the iFrame code of your form, just follow the instruction provided in this article: Getting the Form iFrame Code.

Do let us know if the issue still persists.

    Regards.

    Hi,

The problem is that your form has been embedded between tag. Can you please open your HTML file with your favorite editor and change 124th line from:

    <h2 align="center"><a target="_blank" href="#"><script src="//d2g9qbzl5h49rh.cloudfront.net/static/prototype.forms.js" type="text/javascript"></script>

    to

    <h2 align="center"><script src="//d2g9qbzl5h49rh.cloudfront.net/static/prototype.forms.js" type="text/javascript"></script>

Please let us know if the issue still persists.

    Regards.
