I run a program for our company that requires a 4 page score sheet to be filled out and for each of the 60 categories evidence to support the score needs to be submitted as well. We currently get a binder from each of our 140 dealers sent to us via snail mail and we manually review an score. i am looking for an easier way for our dealers to submit as well as make each submital more equal and easier to review.

Looking at your screenshot above, I do believe that you will be able to set it all up quite simply in the JotForm form builder.
I believe that you will need 2 matrix fields (found under Survey Tools menu) for the scoring, which is easy to do with our conditions or form calculation widget - to be showcased and capture as it is filled out.The following guides are something that I would recommend:Form Calculations How to perform form calculation using the "Matrix Field" Do of course let us know how it goes - but I would suggest creating a free account with us before creating the form so that you do not loose it - since guest accounts are only lasting for as long as the cookies are in place (do keep the URL to the form and we can find it for you even if the cookies are removed).
