I can see the submissions on the back end but I no longer receive e-mails with the PDFs of the forms.
They stopped coming in about a week or so ago.
Please contact me ASAP regarding this.
I was able to locate your form and I see that is currently on an account associated with the username: cfaevents
There are currently two email notifications on the form, once is conditional and is going to firstname.lastname@example.org, the condition is set that the email should only be sent if the date of birth is set to 2015. Which means that mostly likely emails were not being sent out.
If the condition is created incorrectly, please make the change. The other email notification is being sent to: email@example.com
I am seeing that this was recently created and an email notification has already been sent to this address.
