I request to cancel my premium service now and go back to free membership.

I have checked your account and it was already cancelled. With this, you will no longer be billed again and will immediately stop all future scheduled payments for your JotForm subscription. Your account will stay premium until the paid period expires which is on January 24, 2012.
Cancelling your subscription will not affect your forms and data in anyway. It will remain safe when your account downgraded to a free one.
