It was working fine since 2011 but now it's not showing at all. As you can see below, the payment option is in the form editor but missing from the live version. What is going on?Page URL:

It does not longer show because it is conditioned to show based on a response entered in a field which has been already deleted:(see animated image below)
Simply delete that condition, click on Finish and saver your form, in order for the Paypal integration to be shown.

But I've never setup a condition before. Like I said. It was working just fine for years and now in the last couple of weeks a condition pops up? From where?

It could have been there for a long time and was not noticed since it seemed to be working, but as my colleague mentioned, once the field got removed that was causing it to be shown / hidden, the issue seems to have popped out.
If you want, you can take a look at the Form Revision history to see when this might have been added - which might give you a bit more details as who could have added it to your form and when.
