We´ve been having some trouble with the jotform app when sending audits. Most of our merchandisers are able to send the audit without any trouble but sometimes when they send the audit an error occurs (screenshot attached). When this happens I don´t receive the audit.

I have tested your form and I was not able to reproduce mentioned problem. When I submitted your form I got redirected to Thank you page.
I have cleared your forms cache, kindly try submitting again. You can check this documentation on what that error might be related to: MySQL Gone Away Error.
Maybe it was temporarily error. Is there any particular device on which you are getting this error when you submit your form?
