This is actually two forms. After the first one has been filled out, the user is redirected to a second form. Up until this week, we have been receiving both forms just fine.
Beginning this week We are receiving the second form only, but not the first.

Are your referring to the Notification email being sent to the email address - email@example.com for both of your forms (http://form.jotform.us/form/50784755647166 and http://form.jotform.us/form/51675654381159)? I checked the mail logs of email firstname.lastname@example.org and the Notification emails are being sent successfully.
You can also check your form's Email History on your end. You can follow the instruction provided in this article: How to view all your form Email History. Can you try to check the spam folder of your email? The email might be on the spam folder.
