Can I embed a survey form in an e-mail I send from Gmail?

Asked by John Hoff on January 04, 2012 at 09:47 AM

I want to send out a survey form in an e-mail from my Gmail account with a list of questions to which respondents will mostly check 'Yes' or 'No', but in some cases I will want them to make an entry into 1 or 2 addtional data fields.

    Hi John,

You can use your form's source code to embed on your e-mail. To get your form's source code, please follow the screen guide below:

If you can't get it working, then using your Form's direct link is the best solution.

Please let us know if you have further questions. Thank you for using JotForm!
