Why isn't my survey getting displayed on my website?

We use WordPress for our website. I have recently made a new survey. I told my webmaster to upload it to the website. He filled out the correct information, and selected the correct link. When he previewed it on the website, it was not there. 

It looks like the form isn't correctly embedded on your website at the moment. This is the code that I see, and it doesn't look like any of our embed methods:

I would recommend using our iFrame embed codes instead, which should make your form display nicely. Please let us know how it goes, or if you hit any snags along the way, and we will be happy to help you get them sorted.
