I am apart of a travel agency and must collect credit card information from my clients to apply to their trips. It is not something that can use PayPal or the like. My account was shut down and someone told me it might be because I had multiple forms about credit card information. I did not realize you could only have one. What is the best way to get my account back and create a form to collect credit card information from my clients and how much they authorize me to charge.

Unfortunately it is not possible to achieve your requirement.
Please be noted that you should not ask credit card information from your users. If you need to process payments, please use one of our integrated payment tools.

We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities. It's just a matter of time and our anti-phishing filter will disable your form and suspend your account.
I have gone ahead and deleted the credit card related fields from your form and activated your account. Kindly refrain from creating form with email/password, login/password or credit card details.
