The documents that arrive are supposed to be forwarded to my email address and that is not happening. When I try to print the documents off the website, they do not print properly and are not readable.

What is the form? May you be referring to this form: https://www.jotform.com/form/10605543228 ? I see i'ts not properly setup, sender name is missing:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar)
