    Hi,

If you want to embed your form to your Shopify, you can check this article for instructions on how to do this: Adding Form to Shopify Store Frontpage. If you are referring to a different concern, please provide us further details regarding the issue so we can further check?

Do let us know if you need further assistance.

    Regards.

    Hi Chriistian,
I've managed to embed the form just fine. I'm trying to have it such that
the form is sent in the order confirmation email after purchase, however
because it will be after purchase I don't want the customer to have to
enter their information (name and email) again. Is there a way to have the
form pick this up from the checkout info?

    Hi,

Can you give us the link to your shopify page so we check? If you want your customers to receive an email after submitting to the form you can set up an auto-responder. To set up an auto-responder, you can follow the guide: Creating a Form Autoresponder.

    Regards.

    Hi Christian,
Is there a number I can call to speak to someone? What you're suggesting
isn't what I'm looking to do.
I want the customers name and email to *already be attached to/associated
with the form by the time they get it. The link would come to them in the
confirmation of purchase email, which they then enter their measurements in
    for sizing.
    Thank you,
    Erika

I understand that you want to pre-populate customer's name and email in the form link which will be sent on a confirmation email. This can be achieved by adding the information to the URL parameters. Please refer to the guide below that can help you with this.

    http://www.jotform.com/help/71-Prepopulating-fields-to-your-JotForm-via-URL-parameters

You may need to change the information to be filled with the variables pulled from the Shopify website.

Hope this information helps. Let us know if you need any further assistance.
