    form got deleted

    I just found out the appointment request form http://form.jotform.us/jsform/51566522137151

    Got deleted and I am webmaster for the Chiropractor and I am unaware it got deleted.  Your service is freaking awesome and I love how I can design forms but seriously if things just go undeleted for no reason or if you guys choose to delete it then that means lost sales.  I would rather just make a self hosted form for this kind of thing to keep it reliable if that is the case.  I also made one for Signalhillsdental.com and that one is up and hasn't gone deleted yet but I am worrying now.

    I checked the account chiropractor and the form that you mentioned was put to the Trash folder. If there are other user that have access to the account they might have accidentally deleted this form. We do not delete user forms unless it violates our terms and conditions. But we do not delete this immediately, we first disables the account of the user then will manually check the form and then will inform the user that we deleted the form. I have restored the form the you have mentioned. If you encounter this issue again, you can look at the Trash folder and Restore the form.
