I presume that you are looking on how to create the upload field on your form. If that is so, please do take a look at the following guide: How to create upload forms
It will show you how to create the form with the file upload fields.

javonclarkReplied on September 20, 2015 at 12:47 PMI actually want to place a form onto my jotform. So clients completing the jotform can easily access this form. Does this statement make sense? Kind of hard to explain....

If that's a PDF file, you can use this widget to embed the PDF on your form using its direct URL. http://widgets.jotform.com/widget/pdf_embedder
Or this workaround to embed the document using iframe code. http://www.jotform.com/help/153-How-to-Display-a-PDF-Document-On-Your-Form
