why does it say form cant be found?

I checked your account and it seems that your account was automatically suspended. JotForm has a tool called Phishing Detector which allows automatic suspension of forms created for phishing activities. I noticed that you have a form that do not have any field and no form name (http://www.jotform.com/form/52628046618560). This might be the reason why your account was suspended.
