Disappearing formatting?

I spent the better part of the day reformatting an online form I imported from FormsCentral. Finally, everything looked great.  I moved on to set up the autoresponder so that a record of submissions can be sent to the responders. When I went back to the form, all my earlier formatting work was lost! What happened?! Is is possible to recover the previous version so I don't have to do this all over again? How can I prevent this from happening again?

You can retrieve previous changes using Revision History.

An additional advice: please make a clone of the form you just changed, make sure you save all your changes every time.
