I created a form last week, cloning an existing form then modifying it as I've done tons of times before.
The form was used last night, and I received an automated response email listing their selections / inputs.
THEN this morning the form changed back to the initial form that I cloned it from. Zoiks! What up?!
Now I'm looking like a doofus and I'm not collecting the info that I need this week!
The form I created last week was called "HS Youth Service Saturday Sign Up" - see the attached email.
The form I cloned it from is "Clone of Men's Retreat 2014... ".
Help! What's going on?

Our apologies for the inconveniences caused. Is it possible that you are viewing the wrong form? I see that you have both the cloned form http://www.jotform.us/form/52643610060141 and the modified form http://form.jotform.us/form/52597107541154 which appears to have correct information as of checking.
Or it could be the changes you have made was overridden while you are working on the same form on a different tab. You can use revisions history to restore back your form to its previous state. http://www.jotform.com/help/294-How-to-View-Form-Revision-History
