As I tried to check the webpage provided, it says that the webpage is not available. Did you make any changes to the webpage?
I've tried cloning your JotForm to my account and was unable to replicate the issue. However, it looks like the issue with the form itself. As I checked your JotForm, I see that there is a separate condition set to make the upload fields as required. Since the hidden fields are not required to be filled, you may disable this condition and add the required option for these two fields.
I've made the changes on your form and it seems to be validated now. Please check and let us know if the issue still persists. We will be happy to help.

Hi,
I checked and it seems to be validated now.
I need your urgent help to solve the problem with the two cases we discussed in previous messages.
I test the form with the Code Permanent KAZG23588501 or TERA05028706 and I received the message MULTIPLE SUBMISSIONS ARE DISABLED FOR THIS FORM, but I don’t find any submission with this Code Permanent.
I need to add this information!!!!!
If you could look into this I would appreciate it.

I did some tests, but I was not able to reproduce the submission issue. My colleagues have already escalated a bug report ticket to the back end team, so we will need to wait for updates.
