My account has been lost cant log in at all

    I checked your JotForm account in our member database and found that it is currently suspended. The reason why it was suspended is because one of your forms is asking for some sensitive information like account password. This violates the terms of use of JotForm. Note that JotForm does not have account database features that's why a password field is not usually needed.

    You may consider using Profile Manager for such applications instead. Please visit http://interlogy.com/products/pmb/ for more information about this product (from the same guys that gave you JotForm).
